Position Summary :

The Occupational Therapist (OT) will provide rehabilitative, skilled, and medically necessary treatment interventions for patients with cognitive and / or ADL physical functioning disorders, ensuring adherence to all regulatory and clinical practice standards.

Responsibilities :

Screen and evaluate patients with cognitive and / or ADL physical functioning disorders; develop individualized plan of care.

Utilize standardized assessments and evidence-based practices to guide interventions.

Deliver therapeutic interventions including ADL retraining, therapeutic activities, therapeutic exercises, modalities, and assistive devices.

Monitor and document patient progress; modify treatment as necessary.

Provide consultation and counseling to patients, families, caregivers, and service providers.

Communicate effectively with supervisors and interdisciplinary team members regarding patient status and treatment plans.

Prepare comprehensive discharge summaries and maintain accurate records in electronic documentation systems.

Accurately record treatment minutes per regulatory and clinical requirements.

Participate in patient care conferences, utilization review meetings, and facility / team meetings as required.

Supervise / support occupational therapist assistants, aides, and students as appropriate.

Adhere to NBCOT, AOTA, and state practice acts, codes of conduct, and professional ethics.

Submission Requirements :

1+ years of OT experience – Required

LTC or Rehab experience – Required

OT license – Required
