Licensed Practical Nurse (LPN), Skilled Nursing – Days / Mids

Position Title : Licensed Practical Nurse – Skilled Nursing (LTC)

Location : Duluth, MN 55802

Assignment Type : Contract

Shift : 8 & 16 Hour Days / Mids | Variable shifts | 40-hour weekly guarantee

Position Summary

Franciscan Health Center is seeking an experienced Licensed Practical Nurse (LPN) to provide quality nursing care in a Skilled Nursing / Long-Term Care (LTC) setting. The LPN will be responsible for delivering safe, effective, and compassionate care to residents, supporting the interdisciplinary care team, and ensuring compliance with facility standards and regulatory guidelines.

Responsibilities

  • Deliver direct nursing care to residents in a Skilled Nursing / LTC environment.
  • Administer medications and perform treatments as prescribed.
  • Monitor residents’ conditions, perform assessments, and document changes.
  • Collaborate with RNs, providers, and healthcare team members to ensure continuity of care.
  • Assist residents with daily living activities and provide emotional support.
  • Respond promptly to resident emergencies and communicate with providers as needed.
  • Maintain accurate and timely documentation of care, observations, and interventions.

    • Requirements

  • Active LPN License (Minnesota or Compact State).
  • Minimum 2 years of recent LTC / Skilled Nursing experience required.
  • Current BCLS certification .
