The Certified Wound Care Nurse strives to improve patient outcomes via prevention, staff knowledge, and care of patients with ostomies, wounds and / or continence issues. Functions as an expert resource in preventing pressure ulcers and providing wound, ostomy, skin and continence care. Acts as a consultant to assess, plan, implement, evaluate and reassess the care of patients with pressure ulcers, wounds, stomas, or other dermal disruptions. Collaborates with physicians, surgeons, and other care providers to provide evidence based care. Models appropriate behavior as exemplified in MLH Mission, Vision and Values.

What You Will Do

• Performs patient assessments accurately and timely.
• Develops and initiates a plan of care to incorporate patient and family needs.
• Collaboratively monitors the patient plan of care on a continuous basis based upon own assessments and those of other members of the patient care team.
• Demonstrates and promotes professional practice of all members of the nursing care team.
