Registered Nurse (RN) – Home Health

Schedule : Monday – Friday | 8 : 00 AM – 4 : 30 PM (EST)

Coverage Area : Shelby County, TN

Position Overview

We are seeking an experienced Registered Nurse (RN) to join our home health team. This role involves providing skilled nursing care, implementing care plans, and coordinating patient services to promote health, prevent disease, and manage chronic conditions. The RN will work closely with an interdisciplinary team to ensure the highest quality of care for individuals in the community setting.

Key Responsibilities

Conduct health assessments and implement individualized care plans.

Observe, assess, plan, and evaluate nursing support for well, chronic, or acutely ill patients.

Collaborate with physicians, social workers, and other healthcare providers to coordinate care.

Manage and monitor medications, ensuring compliance with all safety and regulatory guidelines.

Document and report changes in patient conditions to physicians and care teams.

Maintain accurate and timely nursing documentation and progress notes.

Supervise and support LPNs and unlicensed staff as needed.

Schedule and coordinate patient appointments and follow-up care.

Provide new employee and annual recertification training for health and safety topics.

Ensure infection control standards and proper maintenance of equipment and supplies.

Requirements

Active RN License (Compact license accepted)

Home Health experience required

OASIS and Homecare Homebase (HCHB) experience preferred

Wound Care Competency knowledge

BLS certification (required)

Preferred Skills

Strong assessment and documentation skills

Excellent communication and time-management abilities

Ability to work independently and manage multiple priorities

Knowledge of home health regulatory and compliance standards
