Requirements : CV, SS Card, Driver's License, NY Infection Control Certificate, ACLS (for NPs; and for physicians requesting moderate sedation), diplomas, CME (past 2 yrs), case logs (past 2 yrs), titers (MMR, Hep B, Varicella), flu shot, PPD, physical, NY medical license, NY DEA, photo, N95 fit test, malpractice / COI documentation, gap explanations, NPDB self-query.

JOB REQUIREMENTS

Licenses : Active NY Medical License, DEA (NY address)

Board : ABEM, AOBEM, ABFM, ABIM (must be boarded within 5 years of residency completion)

Certifications : ACLS (for NPs; and for physicians requesting moderate sedation), PALS (only for Pediatric Hospitalist)
