Your Role as a Physical Therapist (PT, DPT, RPT) in Home Health :

As a Physical Therapist, you will assess and evaluate patient care needs related to functional status, mobility, endurance, pain, and other medical conditions. Your expertise will help shape treatment plans, perform interventions, and evaluate patient progress.

Key Responsibilities :

  • Evaluate patients' functional status and physical therapy needs, assisting physicians in developing therapy plans.
  • Observe and report patients' responses to treatment and any changes in their condition to the supervising nurse or physician.
  • Treat patients to relieve pain, restore function, and maintain maximum functioning using accepted standards of practice.
  • Instruct and inform healthcare team members, patients, and their families / caregivers about the plan of care, functional limitations, and progress toward goals.

    • Licensure Requirements :

  • Current Physical Therapy licensure in the state of OH
  • Current CPR certification
  • Must possess a valid driver's license, vehicle insurance, and access to a dependable vehicle
