Position Details :
- Status : Full Time
- Schedule : Monday through Friday with some weekends and on-call as needed
- Compensation : Per Visit Pay Structure
- Company Vehicle and Gas Card Provided
- Location : Traveling to client homes in the Thumb region and surrounding counties is required
Responsibilities :
Provide skilled nursing care under the direction of a Registered Nurse and according to the plan of careMonitor patients’ health and document clinical observations and vital signsAdminister medications and treatments as prescribedAssist patients with activities of daily living as neededEducate patients and families about ongoing care, medications, and health maintenanceCommunicate and coordinate with the interdisciplinary team to ensure quality care deliveryMaintain accurate and timely documentation in compliance with agency and regulatory standardsRequirements :
Active LPN license in the state of MichiganGraduate of an accredited LPN programPrevious experience in home healthcare or hospice setting preferredCurrent CPR certificationValid driver’s license and reliable transportationStrong interpersonal skills and the ability to work independentlyWhy Join Our Team?
