- Clinical Information :
Covering Emergency Department and Hospital consults and emergencies
- Only ED and Hospital work, no office
- Cover emergencies and any needing surgery
- Patient volume : 0-10 daily
- Required :
CV must be in Month / Year format
- Must have license in hand at time of submission, or proof that the license application has been submitted
- Any gaps greater than 30 days will require an explanation on the CV
- DEA must remain in state to maintain privileges. Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
- Current Flu Vaccine
- Must be fully vaccinated for Covid 19
Exemptions only allowed for religious or medical reasons, once approved by the facility.
- Clean & Active IN License and DEA (or easily transferrable DEA)
- EPIC experience - Preferred
- Board Certified in Urology
- Must be available to work 5 / 5 / 25-5 / 12 / 25
- Required Procedures / Common Diagnosis : Hydronephrosis with renal and ureteral calculous obstruction, Calculus of kidney, Benign prostatic hyperplasia with lower urinary tract symptoms, Calculus of ureter, Urinary tract infection
