About The Position
Job Title : Registered Nurse - Med / Surg TELE

Must-Have :
Experience caring for geriatric patients, preferably in skilled nursing facilities or long-term acute care
Experience with charge nurse duties, dressing changes, and medication administration
Familiarity with enteral feedings and daily nursing assessments
Description :
Immediate start needed.
The facility cares for a variety of patients awaiting placement in the community.
Patients may exhibit heavy behavioral or psychological challenges.
Security is present on the unit due to aggressive behavior from some patients.
The unit mainly provides non-acute care without the need for special monitoring or IV access.
Occasionally, patients may be admitted for IV antibiotics, but they typically do not receive frequent doses.
Patient mobility ranges from independent transfers to requiring two-person assist or Hoyer lifts.
Care is focused on fall prevention, skin integrity, and activities to promote socialization.
Patients are supported by an activities coordinator, certified nursing assistants, and expert care management staff.
Nursing care is framed within a nursing home model.
Vital signs are taken once a day, with full nursing assessments conducted weekly.
Patient ratios follow a team nursing approach with defined guidelines for RNs and CNAs.
Charge nurses are typically unassigned for more flexibility in daily operations.
Daily responsibilities for RNs include medication administration, dressing changes, and charting.
LPNs assist with planning care and administering medications, under specified guidelines.
Certified nursing assistants track daily ADLs, meal intakes, and assist patients to and from activities.
Support staff includes social workers and case managers available for patient needs.
Charting is conducted using an established electronic health record system.
Floating policies are determined by staffing needs and overall patient care.
Weekend rotations are required every other week.
Holiday expectations vary as needed.
Time-off requests go through management for approval.
Scrub color must be professional for all staff, with restrictions on specific items like acrylic nails and certain types of footwear.
