Physical Therapist Home Health

We are seeking a skilled and compassionate Physical Therapist (PT) to join our Home Health team in Harrisburg, PA . This is a full-time opportunity offering competitive pay ($72$80 / hr) and the chance to provide meaningful, one-on-one care to patients in the comfort of their homes.

Key Responsibilities

Perform thorough physical assessments and review medical histories to evaluate patient conditions.

Develop and implement individualized treatment plans tailored to patient rehabilitation goals.

Administer therapeutic exercises and mobility training programs to restore function.

Educate patients and families on injury prevention, wellness strategies, and recovery techniques.

Monitor, document, and adjust treatment progress toward rehabilitation objectives.

Collaborate with nurses, physicians, and other healthcare professionals to ensure coordinated care.

Maintain accurate documentation in compliance with regulatory and agency standards.

Qualifications

Masters degree in Physical Therapy (Doctorate preferred).

Minimum 1 year of home health physical therapy experience.

Current state PT license (Pennsylvania).

CPR certification (must be current).

Valid drivers license and reliable transportation.

Strong interpersonal and communication skills with a patient-first mindset.
