ABOUT THE TEAM ????

The Community Engagement Team assists individuals in their educational, recreational, fitness, and social endeavors with individualized support. They work with each person to create a monthly schedule of well-rounded, meaningful experiences that provide personal enrichment and prosperous employment. People receiving supports spend their entire day in the community and are involved in activities such as working, volunteering, or participating in job-readiness activities.

IN THIS ROLE, YOU'LL : ????

1. Implement Person Centered Plans (PCP) which supports people in achieving their outcomes and goal in

directing their own lives.

2. As part of implementing PCP, assist people in creating monthly schedules to include customized

employment, learning opportunities, volunteering, recreational, and enrichment activities. Work with

program management to identify community resources.

3. Coordinate the schedules of multiple people on the caseload and ensure schedules are followed daily.

4. On a person-by-person basis, support will require the following :

  • Provide assistance with individual personal care needs
  • Provide assistance with individual behavioral needs
  • Provide assistance with individual safety needs while in the community
  • Provide assistance with individual transportation needs (primarily through use of public transportation) including travel training, booking trips, and researching routes.

ABOUT YOU ????

  • Minimum of 1 year experience in supporting people to achieve personal goals or experience of comparable

    • nature that has helped candidate build community-engagement skills

  • Strong analytical skills, good mentoring / coaching abilities and professional approach to personal

    • development

  • Must be willing to learn and use technology such as cell phones, iPhone apps, photos, videos, laptops

    • adaptative technology and case management database.

  • Must be willing to travel from multiple locations throughout Montgomery County as a floater. Using a

    • personal vehicle is strongly preferred as they are following individual's daily schedules

  • Work in various employment, learning, and recreational environments including offices, retail stores,

    • industrial locations, community arts and recreation facilities

  • Must be able to bend, reach, and lift a minimum of 50 lbs.
