Requirements

• Required : must have actual state license in hand, Must be board certified, and must have TX DEA in hand

""Completed applications, updated CV, government photo ID, state medical license and DEA (TX address), DD214 (military), board certification , education and training verification (letter of completion and competency accepted if new resident or graduate), ECFMG (if applicable), case logs (24 months), CMEs (24 months) - 4 stroke education hours annually and 9 trauma for a total of 27 during the 3-year survey period, required life cards per service line, TX CPA, license link for NP / PA, 3 peer references, ACLS (if not Board Certified EM ), ATLS, NIHSS.

Board certified EM physicians preferred but facility will accept FM / IM trained w / ED experience

EMR : EPIC

Physician Coverage : 7a-7p & 7p-7a daily
