Title : Integrated Behavioral Health Provider
Reports to : Director of Care Coordination

Purpose :
Under the general supervision of the Director of Care Coordination and the Chief Clinical Officer, the Integrated Behavioral Health Provider is responsible for delivering evidence-based behavioral health services. As part of an integrated behavioral health and residential practice, their duties include conducting initial patient evaluations, performing triage assessments, managing crisis interventions, providing short-term individual and group counseling, offering skills training, coordinating care, and conducting follow-up sessions.They participate in interdisciplinary case conferences, work with primary care providers, and assist with
program development.
Minimum Qualifications / Credentials :
Master’s degree in social work, counseling, or marriage and family therapy
Must be Licensed Practitioner of Healing Arts (LPHA)
Must be Certified Drug and Alcohol Counselor (CADC)
Two (2) years of progressively responsible experience in mental health services.
Bilingual in Spanish / English is highly desirable.
