    Responsibilities

  • Collaborate with the interprofessional team to delivery high quality person-centered care .
  • Assist patient with accessing community-based resources by supporting them with applications , transportation needs and follow up on any referrals made to ensure patient utilizes identified resources .
  • Enhance patient's ability to effectively communicate with healthcare provider
  • Works with staff and providers to facilitate a care plan and communicates with them regarding the status of efforts
  • Support patient understanding of educational materials and resources about diabetes

    • Support patient understanding of educational materials and resources about diabetes

  • Reinforces standards of care for diabetes including preventative care , annual monitoring and follow up care

  • Reviews medication plan with patient to confirm understanding and makes appropriate connections with prescribing provider when necessary

  • Assists patients with recognition of the barriers and implementation of strategies for healthy eating

    • Qualifications

  • CMA, LPN , Pharmacy Technician, EMT / Paramedic
  • Diabetes Community Care Coordinator Certification required within 60 days of hire
  • Community Health Worker certification within 90 days of hire
  • Mandatory Reporter certification
