We are currently seeking an experienced :
HOSPICE & PALLIATIVE CARE NURSE PRACTITIONER
Per Diem
(Covering Onslow and surrounding counties)
Job Description :
- Implements palliative care program requirements
- Completes medical visit with patients per guidelines and coordinate care with Palliative Care team as clinically indicated
- Conduct face to face encounter to provide re-certification information to Medical Director / Associate Medical Director for Hospice Patients
- Provide clinical findings of that face to face encounter Primary care physician, and / or supervising Medical Director / Associate Medical Director
- Communicate with patients / families at all levels.
- Communicate with all agency personnel
