The Licensed Practical / Vocational Nurse is responsible for providing direct patient care under the supervision of a Registered Nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.
Primary Responsibilities :
- Provides direct patient care as defined in the state Nurse Practice Act and agency's policies
- Implements plan of care initiated by the Registered Nurse
- Provides accurate and timely documentation consistent with the plan of care
- Assesses and provides patient and family / caregiver education and information pertinent to diagnosis and plan of care
- Participates in the coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietician) to the Registered Nurse and / or Clinical Supervisor
- Follows all infection control standard precautions and safety guidelines / standards as per agency policy
- Uses equipment and supplies effectively and efficiently
- Participates in personal and professional growth and development
- Adheres to the Agency's Standard Operating Procedures as it relates to the submission of the documentation
Communication :
Prepares clinical notes and updates the primary physician when necessaryCommunicates with the physician regarding the patients' needs and reports any changes in the patients' condition; obtains / receives physician's orders as requiredCoordinates services and schedules with the Clinical Team Manager (CTM) and Clinical Team Coordinator (CTC / Scheduler) to include recommendations for additional home health care services for patients by the end of the work dayAdditional Duties :
Participates in on-call duties within the on-call rotation schedule, to include weekends as assignedDemonstrates knowledge and observance of Patient Rights and Notice of Privacy PracticesEnsures that arrangements for equipment and other necessary items and services are availableMaintains a daily patient case load and point of care documentation levels, as per Agency standardsRequires the availability of accepting patients during business hours (8 a.m. - 6 p.m.)Requires updating the time card daily, as per UHG PolicyRequires contacting patients the night before the scheduled visit to provide an arrival time within a two (2) hour windowRequires obtaining pre-approval from direct supervisor for overtimeRequires the ability to work flexible schedule to meet patients' needsDemonstrates personal responsibility with regard to attendance and punctualityMaintains privacy and confidentiality with regard to all patient, staff, and agency informationDemonstrates flexibility, enthusiasm and willingness to cooperate while working with others or in place of others, as necessaryExpresses verbal and written communication in a clear, positive and cooperative mannerPromotes the agency's image by adhering to the agency's dress codePerforms all other related duties as assignedYou'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.
Required Qualifications :
High school diploma or equivalentGraduate of an accredited practical nurse or vocational nursing programCurrent unrestricted license in the state of Texas as a practical nurse or licensed vocational nurseBLS / CPR certification2+ years of clinical experience preferable in a community home health or medical / surgical settingDemonstrated ability to lift and transfer patientsAccess to reliable transportation that will enable you to travel to client and / or patient sites within a designated areaTexas Driver's licenseThis position requires Tuberculosis screening as well as proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor's diagnosis of diseasePreferred Qualifications :
