Job Summary :

We are seeking a compassionate and skilled Direct Support Professional (DSP) with a strong background in behavior modification and implementing intervention plans for individuals aged 21 and older. The ideal candidate will be responsible for supporting individuals in achieving their personal goals, enhancing their independence, and promoting positive behavior through effective intervention strategies.

Key Responsibilities :

• Implement individualized support plans and behavior modification strategies tailored to the unique needs of each individual.
• Collaborate with a multidisciplinary team, including case managers, and therapists,to ensure comprehensive support.
• Monitor and document progress toward behavioral goals and report findings.
• Provide direct support in daily living activities, including personal care, social skills development, and community integration.
• Utilize positive reinforcement and evidence-based techniques to manage challenging behaviors effectively.
• Maintain a safe and supportive environment, ensuring the physical and emotional well-being of individuals in your care.
