Key Responsibilities

Conduct comprehensive assessments of older adults to identify their needs

Develop and implement individualized care plans to support independence and quality of life

Coordinate services across healthcare providers and community agencies to ensure seamless support

Required Qualifications

3+ years of experience in geriatric care coordination or related fields

Bachelor's or Master's degree in Social Work, Gerontology, or a related field preferred

In-depth knowledge of community resources and aging services

Experience with long-term care insurance policyholders preferred

Must be 18 years of age or older
