Occupational Therapist Assistant Home Health

Are you an Occupational Therapist Assistant (COTA) looking for a new PRN opportunity? Abode Home Health is seeking a passionate, dedicated Home Health Occupational Therapist to join our team in Pueblo, CO! You will make a difference in our patients' lives by providing safe patient care, delivering individualized physician care plans to ensure an optimal care experience, working as part of an interdisciplinary team, and supporting both patients and their families. Location : Pueblo Branch

Coverage area : Pueblo CO / Fremont County

Schedule : PRN, 15-25 visits / week

Responsibilities

  • Perform ongoing evaluation of patient's functional status
  • Teach and / or retrain patient's skills in daily activities
  • Provide instruction to patient's family / caregiver on treatment regimen
  • Follow treatment program and goals for improved patient function as advised by the Registered Occupational Therapist
  • Maintain appropriate clinical records and submit documentation
  • Ensure compliance with all state and federal legal and regulatory requirements
  • Additional responsibilities as assigned

    • Qualifications

  • Graduate of an occupational therapy assistant education program accredited by ACOTE
  • NBCOT certified with current licensure to practice in state of operation
  • 1+ year COTA experience
  • Home health experience preferred
  • Commitment to clinical and documentation excellence
