Shift : Nights, 5x8 (11 : 00 PM – 7 : 00 AM)

Weekly Hours : 40

Overview

Seeking an experienced LPN / LVN for a Long-Term Care / Skilled Nursing Facility assignment. The nurse will provide compassionate bedside care to residents with chronic or complex health conditions in a supportive environment.

Key Responsibilities

  • Provide direct patient care under RN supervision in accordance with care plans
  • Administer oral, IM, and IV medications as prescribed
  • Monitor and document changes in residents’ conditions
  • Assist with ADLs, mobility, and wound care
  • Collaborate with interdisciplinary team members to ensure continuity of care
  • Maintain accurate documentation in the electronic health record system (CPSI)
  • May be expected to take charge as needed
  • Float within LTC units as required

    • Requirements

  • Experience : Minimum 1 year in LTC / SNF setting
  • Certifications : BLS (Required)
  • Licensure : Active and valid LPN / LVN license (must be eligible to work in PA at time of submission)
  • Systems Experience :

    • Medication System : AHT

  • Documentation System : CPSI
  • Additional : Willing to work every other weekend

    • Patient Population

  • Common Diagnoses : Dementia, Parkinson’s, Impaired Mobility, COPD, CHF
  • Age Group : Adult and Geriatric

    • Additional Information

  • Travelers Accepted : Yes, first-time travelers welcome
  • Float Requirement : Within LTC floors only
  • Call Requirement : None
  • Vaccination : COVID-19 and Flu vaccines recommended but not required
