Practice Manager

The Practice Manager is responsible for leading day-to-day practice management and serves as the manager of the nursing team. As part of onsite management team with the Physician Manager / Lead and Account Management, the Practice Manager guides the clinical and administrative operations of the multidisciplinary, integrated primary care service, which is focused on achieving Crossover's quadruple aim : 1) decreased health care spend, 2) increased quality, 3) improved patient engagement, and 4) strong employee / provider engagement.
Job Responsibilities
- Collaborates and leads with an interdisciplinary leadership team to : improve quality of care; ensure appropriate, safe, and effective quality patient care; and overall clinic efficacy.
- Provides patient care, using Crossover's patient-centered medical home approach.
- Manages a nursing team which supports Primary and specialty care (e.g., RNs, LVNs, MAs, etc.) across multiple onsite clinics
- Partners with the onsite management team, the finance team, and regional / market leadership to develop and implement annual clinic goals and operating plans, including achieving operating system metrics, practice outcomes, growth targets, population analytics, patient operating performance, and financial metrics.
- Collaborates on leading the implementation of new health technologies, products, services, and programs
- Implements / optimizes Crossover's operating system, ensuring quality and compliance goals and metrics are achieved.
- Partners with the onsite management team, the finance team, and regional / market leadership to develop and manage clinic budgets. Identifies and recommends account growth ideas.
- Partners with the onsite management and customer success team to develop plans for maximizing client outreach and increasing clinic utilization.
- Identifies and recommends actions to enhance current services, partners with the onsite management team on developing and implementing new services.
Required Qualifications
RN graduate of an accredited school of nursing with a current state RN licenseMinimum 8 years of clinical experienceCurrent BLS (Basic Life Support) certificationMinimum 3 years of experience staffing and managing a team of employeesMinimum 3 years experience operating and managing a multi-speciality, multi-provider Wellness Center, or equivalentMinimum 3 years of experience utilizing health analytics information - able to measure, analyze and report back key clinical information to patients, care teams, and client representativesPreferred Qualifications
Advanced degree (MS, MBA, and / or MPH) from a recognized college or university, or equivalent senior-level experience is preferred.Experience in applying continuous improvement methodologiesProficiency in healthcare analytic tools, systems, and analysisCertification in Occupational Health (COHN / COHN-S) may be required for some sitesExperience with workers compensation or disability case managementSpecific experience with primary care, physical medicine, coaching, occupational health and other ancillary servicesOutpatient care and urgent care experienceAdvanced training or certification in management of clinic operations.Excellent communication, interpersonal, and organizational skills to serve as a trusted advisor to the client and leader to team members.Excellent computer skills with knowledge of Apple and Google-based technology or client technologies such as Microsoft or HPPassion for delivering a best-in-class patient care experience in an integrated patient-centered primary care modelCreative, flexible and easily adapts to changing prioritiesEnergetic, takes initiative and able to work in a team environment as well as autonomously when neededExhibits a strong work ethic, flexible, innovative, efficient and focused on results / outcomesSets the standard for personal integrity, honesty, teamwork, transparency, and accountabilityPhysical Job Requirements
