Nationwide Therapy Group is seeking a LPN / LVN for a travel job in Bad Axe, Michigan.

Job Description & Requirements

Shift : 6 : 00 PM – 6 : 00 AM (12-hour night shift)

Location : Michigan

Key Responsibilities :

  • Provide supportive services to nurses and staff as assigned.
  • Assist residents with Activities of Daily Living (ADLs) including :

    • Lifting, turning, moving, and positioning in beds, chairs, wheelchairs, and lifts.

  • Bathing, toileting, and feeding assistance.
  • Coordinate dining room services : setup, cleanup, tray delivery, feeding, and documenting intake.
  • Deliver nutritional supplements and ensure proper intake.
  • Weigh residents per facility policy and report weight changes.
  • Monitor skin integrity and report pressure areas, skin breakdown, or tears.
  • Complete daily flow sheets documenting resident care and progress.
  • Report any changes in residents’ conditions to nurses / supervisors.
  • Communicate barriers to performing duties (e.g., resident refusal, reassignment).

    • Submission Requirements :

  • Experience : Minimum 1 year in LTC (Long-Term Care) – REQUIRED
  • Certifications : BLS (AHA) – REQUIRED
  • Travel Experience : Previous travel assignments – REQUIRED

    • Job Details :

  • Weekends : Every other weekend
  • Guaranteed Hours : 36 / week
  • Contract Duration : 13 weeks

    • Certifications Required :

  • BLS (AHA)

    • State Licenses Required :

  • Michigan
