Job Description

American Traveler is hiring an experienced RN Case Manager for a Home Health position in Wisconsin, requiring home health experience and a WI or compact RN license.

Responsibilities

Home Health setting serving patients of all ages in their homes

Responsible for case management including assessment, care planning, coordination, and evaluation across the health care continuum

12-hour day shifts

Travel to patient residences using personal vehicle

May be required to float to other facilities in the Central Region including Wausau, Merrill, and Langlade

Responsible for developing, implementing, evaluating, and revising individual care plans in partnership with an interdisciplinary team

Provides direct patient care, supervises Home Health Aides and LPNs, and coordinates resources for optimal outcomes

Participates in on-call rotation and may cover sick or vacation shifts as needed

Uses best practices for pain and symptom management and acts as patient advocate with physicians

Documents all assessments, care, and communication promptly according to established standards

Expected to participate in interdisciplinary meetings and quality improvement initiatives

Annual flu and other vaccinations may be required by agency policy

Travelers may on occasion be called off up to 36 hours per 13-week contract

Requirements

Active Wisconsin or compact RN license required

Current BLS certification from AHA or Red Cross required or obtained within 90 days of hire

Minimum five years of broad-based nursing experience preferred

Previous home health or hospice nursing experience preferred

Valid driver’s license (Wisconsin or Michigan) and automobile liability insurance with at least $100,000 / $300,000 coverage required

Ability to travel to client homes for care delivery

Diploma, Associate, or Bachelor's Degree in Nursing
