Position Overview
Palo Alto County Health System (PACHS) is seeking a flexible and dedicated PARC Hall Unit Coordinator to support the operations of our long-term care facility. This role plays a vital part in maintaining a clean, safe, and compassionate environment for residents and staff. The Unit Coordinator provides clinical leadership, assists with administrative and support tasks, and ensures continuity of care in the absence of the Director of Nursing.
Key Responsibilities
- Serve as backup MDS Coordinator and assist with clinical documentation.
- Assess resident conditions and initiate appropriate nursing actions.
- Administer resident education and support restorative care goals.
- Supervise long-term care staff when the Director of Nursing is unavailable.
- Receive and transcribe physician orders accurately.
- Administer medications and specialized therapies, documenting reactions and outcomes.
- Assist with clinical rounding and one-on-one resident support.
- Support housekeeping, laundry, and meal service tasks as needed.
- Escort residents to appointments and assist with activities.
- Maintain accurate and timely documentation in electronic / paper medical records.
- Uphold infection control protocols and safety procedures.
- Promote resident dignity and rights through compassionate care.
- Be flexible with scheduling and willing to fill in for nursing and CNA shifts.
- Perform other responsibilities as requested by leadership.
Requirements
Education :
Graduate of an accredited nursing program.Current Iowa RN or LPN license.Completion of LPN supervision course (or agreement to complete within 6 months of hire).MDS certification preferred; MDS training required within 6 months of hire.Certifications :
Current CPR certification.Skills :
Strong communication and interpersonal skills.Basic computer proficiency.Ability to work flexible hours and adapt to changing healthcare environments.Commitment to resident-centered care and professional standards.Maintains certification for assigned work area.Benefits
