Job Title : Physical Therapist Assistant

Description :

We need additional staff to provide in-home services in the fastest growing segment of community health - Homecare.

The Physical Therapist Assistant will maintain a caseload of homecare clients through assessment, plan of care, coordination and implementation of evidence-based care for each client, with the support of an interdisciplinary care team.

This includes but is not limited to appropriate treatment intervention, evaluation of outcomes, and utilization of services to ensure that patient care needs are met.

Responsibilities include clinical assessment and treatment of patients referred by a licensed physician.

Evaluate patients' needs for physical therapy and develop appropriate treatment plans.

Refer patients to other professionals or agencies when appropriate.

Provide appropriate care and treatment to patients based on the physician care plan, agency policy, and professional standards of care.

Coordinate and communicate treatment activities with healthcare team members and the patient.

Instruct family members, physical therapists, home health aides, and other caregivers in appropriate physical therapy techniques.

Contribute to the professional development of new staff and health professional students.
