$7,500 Sign-On Bonus
POSITION SUMMARY
Under the direction of the Chief Medical Officer, and in accordance with CHOT policies and procedures, is responsible and accountable for collaboration with CHOT physician(s), and other care team members, in an active practice to deliver palliative care and hospice related health care services to patients and families.
ESSENTIAL FUNCTIONS
- Documents medical history, pertinent patient information, hospice eligibility, disease progress, functional and physical decline, medications, and / or diagnostic studies as required for the patient visit (per agency documentation standards); communicates findings and changes to Medical Director.
- Consults Medical Director or designee when plan of care is outside of standardized practice and protocols, and specialty consults are needed to manage patient’s care.
- Reviews, follows, and / or updates written plan of care in coordination with an interdisciplinary Team (IDT).
- Assures that patient receives appropriate measures to control symptoms, through collaboration with IDT members.
- Document care provided in the patient’s clinical record providing evidence of the progression of the end-stage disease process.
- Attends Physicians meetings and other meetings as directed by Medical Director or Director / Senior Director.
- Establishes, builds, and nurtures relationships with staff and community referral sources to facilitate program growth.
- Participate in the QA / PI process as required.
- Perform all duties within the scope of local, state, and federal regulations pertaining to care and documentation for all payers in palliative and hospice care.
- Perform other related duties or special projects as directed by the Director / Senior Director and / or Medical Director.
POSITION QUALIFICATIONS
Registered Nurse duly licensed to practice as an Advanced Practice Nurse in the state of Texas under the Texas State Board of Nurse Examiners provisions. Prefer additional accreditation from an approved certifying body for advanced practice nursing (American Academy of Nurse Practitioners, American Nurse Credentialing Center, or HPNA Advanced Certification in Hospice and Palliative Nursing ACHPN).Comprehensive knowledge and well-developed skills in :Medicine and Pharmacology
Pain and Symptom controlPsychology of lossUnderstanding and acceptance of Hospice principles of care.Requires a minimum of 3 years nursing experience, preferably at APN level.Valid Texas Driver’s LicenseProof of current Automobile Liability InsuranceProficient skill using a computer is required; Microsoft office suite experience required.Reasonable Accommodations Statement
