Developmental Disability Support Coordinator Manager

Overview

ROCKBRIDGE AREA COMMUNITY SERVICES RACS is currently seeking a motivated, qualified individual to work in our coverage area of Rockbridge and Bath County, and the Cities of Lexington and Buena Vista. RACS will provide training, supervision, and education opportunities specific to the position.

We are seeking a DD Support Coordinator Manager to supervise the Developmental Services Unit DD Support Coordinators and oversee all services provided within the program. The incumbent will screen and disperse referrals as they are received.

Responsibilities

• Ensure the team is in compliance with regulatory entities (including DBHDS Licensing, Department of Justice, and Medicaid), along with all agency policies and procedures.
• Must remain current in reference to any regulations and / or form changes that are initiated by DBHDS and / or DMAS.
• Facilitate and encourage the establishment of standardized processes in Service Coordination programming, including intra- and inter-Agency processes, with the goal of increasing efficiency, increasing productivity, and ensuring the minimization of any real or perceived conflict between Waiver Services Programs in collaboration with Targeted Case Management.
• Ensure all monthly contacts are made and the unit meets all established billing goals.
• Review all electronic health record reports and clinical transactions for each Support Coordinator on a regular basis, both throughout the month and at the end of each month.
• Represent the Unit at both FAPT and Regional DD Coordinators meetings and other Stakeholders\' meetings as appropriate.
• With input from the Director, help to develop the program\'s revenue target for each year.
• Must be able to work independently, demonstrate initiative and strong team-building skills, as well as be able to demonstrate the ability to problem-solve behavioral issues and emergency situations.

Probationary Period
