Summary
Kansas Palliative & Hospice Care is seeking a compassionate and collaborative Extended Care Liaison to provide extended evening and weekend support to patients and families, while serving as a strategic connector between clinical care and sales outreach. This role blends direct patient presence-including vigil care for actively dying patients-with early hospice identification, account coordination, and community-based programming across SNF, AL, and Memory Care settings.

Qualifications
Supervisory responsibilities : This position does not involve any direct supervisory responsibilities.
Budgetary / Fiscal Responsibility : Responsible for using resources wisely to support cost-effective operations.
Compliance Responsibility : As an essential function, this position is responsible for complying with the Compliance Program as it applies to the individual job duties, the department, and the company. This position will exercise due diligence to prevent, detect, and report unlawful and / or unethical conduct by fellow co-workers, professional affiliates, and / or agents.
Policy and Procedure Interpretation and Development : Responsible for complying with all company and clinical policies and procedures. Participates in audit process as requested to ensure compliance with policies and procedures. Maintains confidentiality of privileged information and supports the employee's and patient's right to privacy.
Work Environment / Schedule / Travel : Routine schedule during standard business hours. Hours worked are based on the assigned patients' needs. On call shifts as scheduled. Requires frequent travel to and work in patients' homes within hospice's service area, and attendance at meetings in office as needed.

Considerable reaching, stooping, bending, kneeling and / or crouching. Visual acuity and hearing to perform required care tasks. Frequent driving of own vehicle in various weather conditions. Potential exposure to virus, disease and infection from patients and specimens in working environment. Prolonged or considerable sitting or standing to perform work on a computer, telephone, or other equipment.
