Overview
We are seeking a Post Adoption Case Manager to join enCircle . If you are passionate about working with adoptive and kinship families, have experience in the child welfare field, and possess trauma-informed skills, apply for the role of Post Adoption Case Manager at enCircle ! You will be serving youth and families in Newport News and the surrounding area, with paperwork expected to be completed remotely. This position is located in Newport News, Virginia.
Responsibilities
- Provide trauma-informed case management and supportive services to adoptive and kinship families, reporting to the Regional Supervisor.
- Coordinate and provide services to children and families using a trauma-informed and strengths-based model; deliver best practice and evidenced-based services for working with youth.
- Provide family support to increase stability and promote healing relationships for youth.
- Conduct virtual and occasional in-person home visits and phone contacts to assess needs and connect families to services addressing risk factors.
- Create and monitor service plans with families, linking and coordinating services including information and referrals, parenting education, and training; ensure plans are trauma-informed and strengths-based.
- Connect families to adoption-competent mental health services and other community supports; advocate for increased family support.
- Develop discharge planning and resources for future access.
- Engage in ongoing case management with screening and referrals to address a child's needs; regularly evaluate and adjust plans.
- Provide crisis intervention directly or connect families to services to ensure safety during crises.
- Submit thorough documentation in compliance with policies and procedures; use social work best practices for assessments of child progress and services provided.
- Maintain regular communication with supervisor about safety, stability, and well-being in placements; participate in ongoing supervision.
- Perform other duties as assigned.

Bachelor's degree in social work or a field related to social work such as sociology, psychology, special education, or counseling.A minimum of three years of experience with child welfare programs such as foster care, foster care adoptions, adoptions, or post-adoption services.Knowledge of community resources and ability to connect families to community services.Ability to maintain records, prepare reports and required paperwork according to licensing and agency requirements on a timely basis.Ability to work effectively with people form diverse backgrounds.Ability to work a flexible schedule to include weekends and evenings on occasion.Excellent crisis intervention skills.Excellent verbal and written communication skills.Critical assessment and analysis skills.Proficiency in Microsoft Office applications.Ability to travel.Valid driver's license required.Must be able to pass an FBI background check as well as a CA / N check.Ability to acquire Crisis De-escalation, First Aid, CPR Certification, and other necessary trainings as required.Ability to observe health and safety regulations.Specific Duties And Responsibilities
Conduct virtual and occasional in-person home visits and phone contacts with the purpose of assessing child and family needs and connecting the family to services in the community to address risk factors.Create a service plan with the family to link and coordinate services. Services may include information and referrals, parenting education, and training. Develop service plans that are trauma informed, and strengths-based and provide guidance and direction.Connect families to available adoption competent mental health services and other supports in the community. Advocate with community resources to increase the family's support.Create discharge planning to provide the family with resources and contacts that the family can access in the future.Ongoing case management with a focus on screening and making appropriate referrals to address a child's needs.Continually evaluates and reviews each family's plan.Provide crisis intervention services directly, as needed, or connect family to services to ensure minor's safety and well-being in a crisis.Submit detailed and thorough documentation in compliance with policies and procedures.Utilize social work best practices for thorough assessments of child's progress and services provided.Maintain regular and timely communication with supervisor around any issues concerning the child's safety, stability, or well-being in the placement.Participate in ongoing supervision.Other duties as assigned.EEO and Accommodations
