Position Summary
Join Providence at Home with Compassus as a full-time Palliative Care Nurse Practitioner. The Nurse Practitioner will be the main provider for the community-based Palliative Care program for adult patients. The program was initiated in 2021 and has a current census of 85. In this position, the NP will make in-person home visits with oversight from the medical director and program director. Follow-up visits will be made according to need, with a cadence of every 3 to 12 weeks. The work will involve only out-patient (home based) palliative care, and the NP is expected to collaborate with the patient’s PCP and specialists. The NP can prescribe medications, including controlled substances, as appropriate and indicated. An MSW and nurse is also a member of the palliative care team, and the NP will work closely with this individual as well as the program director / manager.

Physical Demands and Work Environment : The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential. Compensation range : $56.93-$73.67
The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby / on-call, overtime, premiums, extra shift incentives, or bonus opportunities
