Description :

Rehabilitation therapy position available for a Certified Occupational Therapy Assistant.

Candidates should possess a valid state license.

Experience with Rehab Optima is preferred.

Candidates are required to complete a comprehensive credentialing checklist prior to submission.

Successful applicants will need to provide a resume and three references.

Criminal background checks will be conducted, including county, federal, multi-state, and nationwide searches.

A health and human services search will also be performed.

A social security number trace is required.

Candidates must provide proof of tuberculosis screening, either via PPD or Quantiferon, or via a positive PPD result accompanied by a chest X-ray.

Completion of a SAM search is necessary.

All candidates must comply with Iowa SING background requirements.
