The Program Manager of Readmission Prevention Strategies provides leadership that supports AdventHealth's strategies in readmission reduction. In partnership with AdventHealth Care Setting team and AdventHealth leadership, this role engages with stakeholders in the planning, design, and execution of readmission reduction initiatives. Accountable for driving outcomes and achieving goals. Ensuring strategic alignment and executive engagement for readmissions prevention for AdventHealth. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

The value you'll bring to the team :

• Oversees HRRP initiatives, ensuring alignment with organizational goals and regulatory standards.
• Manages work groups, including Clinical Integrated Network, Acute, and Post-Acute Services, to drive performance.
• Collaborates with Physician Leaders, CMOs, Institutes, and ACS to develop and implement readmission prevention strategies.
• Partners with care transformation teams and physician leadership to enhance quality, reduce costs, and improve consumer experience.
• Builds and maintains effective relationships with internal and external stakeholders to coordinate activities and resolve issues related to readmission strategies.
• Reviews, prepares, analyzes, and presents reports and recommendations to senior leadership for informed decision-making.
• Aligns readmission strategy with stakeholders such as Institute leaders, CIN leaders, and AHS-PHSO.
• Ensures adherence to Corporate Compliance Plan and all relevant regulations.
