Acentra Health is looking for a RN Clinical Assessor to join our growing team.
Job Summary
The purpose of this position is to complete needs-based eligibility determinations for patients who are applying for Medicaid-funded personal care services provided in their home or in adult care or supervised living homes. This position also assesses level of care (LOC) to allow targeted individuals to remain in or return to a home and community-based setting.
- This is a hybrid position that includes both remote work and fieldwork within the Hillsborough County area of New Hampshire, specifically within a 30-mile radius of Manchester, to ensure effective coverage of the catchment area. Travel will be necessary for home visits, as well as visits to hospitals and nursing facilities. We offer travel reimbursement at the current federal mileage rate, in addition to covering tolls and parking expenses.
Responsibilities
Conduct assessments to determine whether the beneficiary meets the conditions and criteria for PCS eligibility, using state-approved standardized assessment tool(s)Ensure PCS are provided on a "needs basis" in quantities appropriate to the Beneficiary's unmet need for services based on the severity of their medical condition, functional disability, physical, or cognitive impairmentEnsure the privacy and dignity of individuals receiving assessment for PCS is maintained at the highest standardsEnsure that new, expedited, annual, change of status, mediation / appeals, reconsideration review, and derivative assessments are conducted within established timeframesInclude an interview with family members and informal caregivers who are present at the time of the assessmentProvide the Beneficiary with guidance and assistance, as necessary, to select PCS providersConduct service plan reviews as neededSubmit the completed assessments using state-approved interfaceParticipate in the Beneficiary's mediation and appeal processesRespond to state inquiries regarding assessments conductedAttend and actively participate in staff meetings and conduct case consultations / peer reviews / internal auditing as assignedProvide assessments for initial eligibility determinations for an applicant to participate in a 1915(c) HCBS program, and, when applicable, annual and change of status assessments for participants currently participating in a 1915(c) HCBS program, using state-approved standardized assessment tool(s)Read, understand, and adhere to all corporate policies including policies related to HIPAA and its Privacy and Security RulesQualifications
Required Qualifications
Active, unrestricted Registered Nurse (RN) license for the State of New Hampshire or compact licenseGraduation from an accredited college or university with a minimum of an associate's degree in a relevant fieldProficiency in medical record abstracting, including a strong understanding of medical record organization, medical terminology, and disease processesDemonstrated expertise in clinical assessment and critical thinking, ensuring accurate evaluations and recommendationsProficient with Microsoft Office applications, including Excel, Word, PowerPoint, Teams, Outlook, and other web-based softwareFlexibility to adapt to changing priorities and strong organizational skills to manage multiple tasks effectivelyReliable transportation and a clean motor vehicle report (MVR)Preferred Qualifications
2+ years of experience in a home care settingExperience with public mental health and medical programs, as well as public sector services and supportsKeen attention to detail, with the ability to prioritize and execute tasks efficiently and in a timely mannerExceptional verbal and written communication skills, with the ability to convey complex information effectivelyProven ability to work collaboratively within a team environment, fostering cooperation and shared goalsWhy us?
