job_description.job_card.job_descriptionThis position Provides patient / family education.Participates in family conferences. Responsible for all necessary documentationas required by accepted medical guidelines. This position (Outpatient / Inpatient) works under the supervision ofDirector. Evaluates, plans and implements treatment programs for therehabilitation of patients, as directed by a provider, using approved treatmentprocedures. This position works under the supervision of the Director and is employed by Mosaic Health System. Evaluates patients referred by providers. Establishes goals that maximize the patient's recovery. Plans a treatment program, using appropriate treatment procedures that will best accomplish the established goals. Implements the treatment program using approved procedures. Documents treatment procedures and the patient's response to them, on a regular basis. Other duties as assigned All required education is a minimum requirement. Higherlevels of education are acceptable. Bachelor's Degree- Graduate of accreditedschool of physical therapy required. Licensed Physical Therapist in State of Missouri required.Heart Saver certification or BLS required.
