Key Responsibilities :

Provide nursing care to patients in their homes as prescribed by the physician
Coordinate total care plan with other healthcare professionals
Make initial and comprehensive evaluations, develop plans of care, and submit accurate documentation
Ensure patients meet eligibility guidelines

Requirements :

RN licensure in state of practice

CPR certification

Drivers license, vehicle insurance, and access to a dependable vehicle or public transportation
