Registered Nurse (RN) - Med / Surg 3N - PT

We are hiring a Registered Nurse (RN) Med / Surg - 3N - PT for one of our clients in Geneva, NY.

Job Description :

The Registered Nurse applies knowledge of nursing theory, technical, and communication skills in delivering high quality compassionate care to patients and families. The Registered Professional Nurse assesses patient need, develops a plan of care in collaboration with the interdisciplinary team, provides direct care, evaluates patient progress, and coordinates the care delivery throughout the patient's episode. The RN on 3 North develops specialized skill in the area of patient admitted for medical conditions. He / She provides care to adult, adolescent, and geriatric populations with medical conditions. The 3 North RN is accountable for patient family education to assist in understanding self care beyond hospitalization, including lifestyle changes to support health and healing. The 3 North Registered Nurse is expected to promote comfort and provide emotional support in a Healing Environment.

Job Functions :

• Applies nursing knowledge and skill in the delivery of care.
• Assesses patient / family needs through active listening, social perceptiveness (being aware of others' reactions and understanding why they react as they do), and analyzing data or information obtained through direct observation / assessment, technology, or health team communication.
• Creates a developmentally and age appropriate, culturally relevant, individualized plan of care to address the nature and scope of the health problems identified.
• In collaboration with the patient, family, physician, and healthcare team members, sets goals and monitors the patient's progress.
• The RN adjusts the plan based on patient response or change in condition / reprioritization of needs.
• Promotes, advocates for and protects the health, safety and rights of the patient.
• Encourages and builds mutual trust, respect, and cooperation among team members to accomplish the best possible patient outcome.
• Teaches others as appropriate and delegates tasks and interventions consistent with the individual's education, training, and scope of practice.
• Understands the personal responsibility to safeguard public safety through compliance with infection prevention strategies.
• Motivates the patient / family by teaching them what is needed to continue care / healing at home.
• Assesses the patient / family learning styles and structures the sessions to maximize understanding.
• Seeks "teachable" moments during the care delivery process to emphasize pertinent information.
• Asks questions to evaluate retention of key points in the self care process.
• Provides tools and information to assist the patient's family after discharge.
• Documents the education provided and the patient / family response to the education.
• Systematically evaluates quality and effectiveness of Nursing Practice and uses the results of quality improvement activities to initiate changes in individual nursing practice and care delivery.
• Engages in self-evaluation of practice, identifying strengths and areas in which professional development would be beneficial.
• Takes accountability to increase knowledge and enhance skill.
• Uses logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions, or approaches to problems.
• Nursing practice reflects the importance of making a meaningful connection with patient / family.
• Seeks to develop a helping, trusting, and caring relationship by being present in the moment of interaction, listening, communicating, engaging, and anticipating the patient / family needs.
