JOB RELATIONSHIPS

Responsible to Director of Case Management. Consults with members of the Patient Care Services Leadership and Hospital Leadership staff as appropriate.

DEFINITION OF POSITION

The Case Manager will : Assess, plan, implement, coordinate, monitor and evaluate healthcare services to meet an individual’s specific health care needs in a cost effective manner, Monitor the appropriateness and medical necessity of health care for patients while working closely with third party payers. Coordinate patient care with patient, family, staff, physicians, and ancillary / community services to promote quality and cost efficient care.

QUALIFICATIONS

Professional

Registered nurse with current North Carolina license (BSN preferred).

Current BLS Healthcare Provider certification.

Three years clinical experience in a health care facility with knowledge base of clinical nursing principles

Knowledge of current trends and issues related to utilization and case management principles related to the hospital practice setting.

Computer experience

Evidence of participation in ongoing continuing education programs.
