Well Care Services is a Behavioral Health Clinic looking for Case Managers to join our team. We are hiring a team of case managers that will be working hands on with patients out on the field.

Requirements :

Bachelor's degree in Human Services or related field and a minimum of two years of experience working with adults with serious mental illness; or an equivalent combination of training and experience. Knowledge of serious mental illness, substance abuse, psychotropic drugs, entitlement programs and recovery oriented services required. Interviewing, service planning, consultation, public relations, and experience with electronic medical record. Computer skills and the ability to type required. Good driving Must not reflect a total accumulation of six or more negative points within the past three years or a major violation within the past four years.

Duties :

Well Care Services is seeking a full time qualified Mental Health Case Manager to provide services to consumers with mental illness. Functions include assessing consumers' strengths and needs; developing individualized service plans; completing services assessments; linking consumers with other community resources; coordinating services including health care and monitoring sufficiency of services and goal attainment; collaboration with other professionals. Perform other work as required.

Must be able to pass a drug and background check.
