Location : Indiana (Statewide travel required)

Position Summary

The Youth Services Coordinator will play a critical role in supporting children and youth in Indiana's foster care system. This individual will help young people and their caregivers navigate healthcare services, connect with community resources, and address socio-economic challenges that impact long-term stability and wellbeing. The coordinator will collaborate closely with Community Health Workers (CHWs) across the state to ensure youth and families have access to the right care, housing, food, education, and workforce development opportunities. This position requires regular statewide travel. Candidates must have access to a personal vehicle (mileage reimbursement provided).

Key Responsibilities

• Serve as a primary point of contact for foster youth and their caregivers to connect them with healthcare, social, and community-based services.
• Support youth in navigating healthcare systems, including Medicaid and behavioral health services.
• Conduct SDoH assessments to identify barriers such as housing, food security, transportation, and financial strain.
• Develop and maintain resource networks with community partners, state agencies, and nonprofit organizations.
• Work with CHWs statewide to coordinate services and ensure continuity of care for youth.
• Provide care coordination and follow-up support to ensure that referrals result in successful connections.
• Advocate for youth and families, helping them understand and access their rights and available benefits.
• Collect and track program data, documenting outcomes to support program improvement and reporting requirements.
• Promote a safe, supportive, and culturally competent environment for youth and families.

Qualifications Required :

Experience working with youth, foster care systems, or vulnerable populations.
