Certified Occupational Therapy Assistant – SNF

Location : Fork, SC

Schedule : Full-time

Setting : Skilled Nursing Facility (SNF)

Overview :

We’re seeking a dedicated Certified Occupational Therapy Assistant (COTA) to join a skilled nursing facility team in Fork, SC . This role provides the opportunity to assist residents in improving their independence and quality of life through personalized therapy under the supervision of an OT.

Responsibilities :

Assist in delivering occupational therapy treatments and interventions in the skilled nursing setting

Implement individualized care plans developed by the supervising Occupational Therapist

Collaborate with interdisciplinary team members to support comprehensive patient care

Maintain accurate documentation following state and facility guidelines

Educate patients and caregivers on therapeutic exercises and strategies to promote daily function

Qualifications :

Active South Carolina COTA license

Experience in skilled nursing or geriatric care preferred

Strong communication and teamwork skills

Ability to work independently while following clinical direction
