We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our long-term care facility. The LPN will be responsible for assessing, planning, and implementing nursing care for residents under the supervision of a Registered Nurse (RN). This role is crucial in ensuring the continuity of care for our residents by providing direct care and supervising the care given by CNAs and supportive staff members. Additionally, the LPN will actively participate in resident and family education while serving as a resident advocate in alignment with the facility's mission.
Key Responsibilities :
Assess, plan, and implement nursing care plans for residents in a long-term care setting, following the supervision of an RN.
Monitor and evaluate residents' conditions, identifying changes in status and taking appropriate actions.
Administer medications and treatments as prescribed, ensuring accurate documentation and monitoring of effects.
Supervise and guide Certified Nursing Assistants (CNAs) and other supportive staff in providing quality care to residents.
Participate in resident and family education, promoting health, wellness, and disease prevention.
Collaborate with the interdisciplinary team, including physicians, therapists, and social workers, to ensure comprehensive care.
Maintain accurate and timely documentation of resident care, following facility policies and regulatory guidelines.
Serve as a resident advocate, prioritizing resident rights, privacy, and dignity.
Stay current with nursing best practices, regulations, and industry trends to provide high-quality care.
Qualifications :
Current Licensed Practical Nurse (LPN) licensure in the state of practice.
Previous experience in long-term care or skilled nursing facility preferred.
Strong knowledge of nursing principles, practices, and procedures.
Excellent communication and interpersonal skills, with a focus on compassion and empathy.
Ability to effectively lead and supervise CNAs and other supportive staff members.
Familiarity with electronic medical records and documentation.
