What You'll Do :

• ️ Develop and implement service plans to support family stabilization
• ️ Conduct weekly home visits and outreach activities
• ️ Facilitate healthy interpersonal relationships within families
• ️ Advocate for clients and assist with school adjustment and permanency planning
• ️ Attend and participate in multi-disciplinary staffings and court hearings
• ️ Maintain accurate and timely case records and progress notes
• ️ Collaborate with community agencies to ensure wraparound services
• ️ Participate in training, seminars, and professional development

What We're Looking For :

🎓 Education : Bachelor's degree in social work or a related field (Master of Social Work or equivalent preferred)

📌 Experience : Professional experience in child welfare or family services preferred

🚗 Requirements :

Successfully obtain Intact Specialty Certification

Valid Illinois driver's license and proof of required auto insurance

Ability to pass a background check

Strong communication, organizational, and interpersonal skills

Ability to work independently and as part of a team

Comfortable with home-based services and on-call duties
