A community-based outpatient mental health program is seeking a Substance Abuse Specialist to join its Assertive Community Treatment team. This is a clinical role providing recovery-focused, harm-reduction services to individuals with co-occurring mental health and substance use conditions.

About the Opportunity :

  • Schedule : Monday to Friday
  • Hours : 35 hours / week
  • Program : Assertive Community Treatment (ACT)

Responsibilities :

  • Manage a caseload assigned by the Team Leader
  • Conduct community-based individual and group counseling with a focus on harm reduction and recovery
  • Complete all assigned field visits within the standard 7-hour service day
  • Transport clients when needed (e.g., from hospitals or court appointments)
  • Facilitate substance use-focused group sessions tailored to client treatment plans
  • Complete initial and ongoing substance abuse assessments; adjust interventions accordingly
  • Clinical Documentation & Assessments for Treatment plans, risk assessments, wellness / safety plans
  • Submit progress notes on the day of service
  • Complete discharge planning and conduct 30, 60, and 90-day follow-ups post-discharge
  • Assist with administrative and team support
  • Coordinate with medical staff to ensure timely documentation post-hospital discharge
  • Maintain a real-time Outlook calendar with daily visits and mileage
  • Report any client incidents or critical events in line with agency protocol
  • Perform other duties, as needed
  • Qualifications :

  • 1+ year of experience in a Clinical and / or Rehabilitation role / setting
  • Master's Degree in Social Work, Mental Health Counseling, Psychology, or Rehabilitation Counseling
  • At least 1 year of clinical or rehabilitation experience
  • Solid assessment and documentation skills
  • Experience with Mental Health illness affected populations and / or Substance Use Disorder individuals
