Job Description
Job Title : Care Management RN
Location : Remote
Contract duration : 12 months
Schedule : 8am to 5pm (M-F)
Overview :
The Case Manager RN coordinates a multidisciplinary team to deliver holistic, member-centered care. They serve as the primary contact for members, caregivers, and providers, using phone, email, text, and digital platforms. The RN applies the case management process to assess needs, create care plans, coordinate resources, and support improved health outcomes.
Key Responsibilities :
- Lead coordination of care with medical, behavioral health, pharmacy, and community teams.
- Assess member health, psychosocial needs, and support systems.
- Develop and monitor individualized care plans, addressing barriers and gaps.
- Arrange and coordinate resources (community, mental health, financial, etc.).
- Provide education on self-management, medications, nutrition, and post-discharge care.
- Document interactions, monitor progress, and adjust plans as needed.
- Advocate for members and promote self-advocacy.
