Shift : 8 : 00 AM – 4 : 30 PM

Schedule : M–F, 40 hours / week , Every Other Weekend Required

Guaranteed Hours : 40 hours / week

Contract Length : 13 weeks (91 days)

On-Call : No call required

3+ years Home Health RN experience – Required

BLS (AHA only) – Required

Job Description & Responsibilities :

Complete patient assessments and develop care plans

Deliver skilled nursing in patient homes, based on OASIS assessments

Utilize HomeCare HomeBase and Point of Care documentation

Educate patients and caregivers

Collaborate with LPNs, PCTs, and interdisciplinary teams

Must be flexible with schedule changes and weekend rotations

Key Skills :

Strong knowledge of OASIS, care planning, and HH documentation

Time management and autonomy

Communication with families and care teams

Clinical judgment in independent home settings
