Your Role as an RN Case Manager :

• Travel within a 40-mile service area radius of the Bedford / Lynchburg office location for patient visits with generous mileage reimbursement paid out bi-weekly.
• Coordinate and oversee care for hospice patients, ensuring comfort and dignity
• Develop individualized care plans and collaborate with an interdisciplinary team
• Provide education and emotional support to patients and families
• Advocate for patients' needs and ensure high-quality, compassionate care
• Provide direct patient care in homes and contracted facilities
