ACCOUNTABLE FOR :
The Certified Nursing Assistant (C.N.A.) is responsible for maintaining quality of care for all residents.
GENERAL PURPOSE AND ESSENTIAL FUNCTIONS :
Responsibilities and Duties :
Review Activity of Daily Living sheets at the beginning of each shift to ensure that the plan of care if followed for each resident
Document on the ADL sheets as appropriate; document any changes observed with the residents, who you reported the changes to, any resident complaints, any pertinent information that is relevant to maintaining quality of care.
Document appropriately on the Dietary Log, Bath Log, Snack Sheets, Weekly Blood Pressure Sheets, ADL sheets, etc...
Report bowel movements and intake and output to the charge nurse at the end of each shift
Assist with admissions and inventory residents' personal belongings.
Answer call light signals as quickly as possible.
Assist residents to be clean, neat, and well-groomed daily.
Change bed linens on bath days and as needed.
Make beds daily
Oral care every shift and as needed.
Nail care on bath days and as needed.
Incontinence care at least every two hours and as needed.
Catheter care every shift and as needed
Assist with bathing per schedule and as needed
Assist with toileting at least every 2 hours and as needed
Skin care every shift and as needed.
Shave men and women on bath days and as needed
Assist with meals; pass trays; set-up meal trays; feed as needed; pick up trays; clean resident's clothing & chairs after meals.
Document meal percentage intake
Report any coughing, watering eyes, or choking which may indicate a swallowing problem.
Offer snacks, ice, water,drinks morning, afternoon, night, and as indicated.
Ensure that drinking cups are clean; if not, obtain a new one.
Turn and reposition residents who are in bed, wheelchairs, geri-chairs, etc...at least every two hours and as needed
Clean equipment as assigned and as needed (chairs, walkers, side rails, cushions, mats, restraints, etc...
Assist with range of motion exercises, ambulation, walk to dine, toileting, sit to dine, toileting, transfers, etc...
Assist with activities daily
Obtain vital signs as instructed.
Related Functions :
Arrives for the scheduled shift no earlier than fifteen (15) minutes before the shift begins and clocks out no later than fifteen (15) minutes after the scheduled shift ends.
Required to work weekends on a rotating basis.
Responsible for one overtime shift a month if needed.
Demonstrates loyalty to the organization, honesty, and integrity at all times in the care and use of patient / resident and facility property.
Demonstrates positive support of any and all programs / projects implemented by the DON or Administration.
Maintains confidentiality of patient / resident information.
Displays a neat, clean, and professional appearance at all times.
Cooperates when needed if the facility faces an emergency or unusual circumstance.
Participates in Continuous Quality Improvement activities through monitoring and evaluation of patient care.
May be asked to participate on committees and community projects.
Participate in staff meetings, in-service programs, performance improvement, infection control activities as necessary.
Promote teamwork with physicians, patients, families, visitors, and co-workers.
Use proper body mechanics for transfers, lifting, etc...
All other duties as assigned.
REQUIREMENTS :
Education / Training Experience :
High School Diploma or GED equivalent. Must be state certified and be on the state registry. At least one year of C.N.A. / P.C.T. experience preferred.
Freedom from illegal use of drugs and freedom from use and effects of drugs and alcohol in the workplace. At least one year of clinical experience preferred.
Verbal and Written Communication Skills :
Must be able to read, write and speak the English language fluently. Must be able to relate to individuals with various ethnic, cultural, and socioeconomic backgrounds.
Interpersonal Skills :
Relates cooperatively and constructively with all patients, guests, families, physicians, peers, and all healthcare members as well as anyone that JHS' employees interact with while performing their daily duties.
Essential Technical / Motor Skills :
Full Ambulation and precise eye-hand coordination.
Essential Mental Abilities :
Knowledgeable and competent to perform essential functions. Must be able to deal with a variety of personalities with whom the employer comes into contact with, both pleasant and irate.
Essential Sensory Requirements :
Ability to visually and audibly perform all essential functions.
Essential Physical Requirements :
Moderate to high amount of walking, standing, sitting, reaching, pulling, pushing, stooping, and kneeling. Minimum lifting requirement of 50 pounds.
Exposure to Hazards :
Exposure to infectious diseases, blood and body fluids, and chemical products.
Other :
Assume responsibility for professional growth and expertise.
Standards of Performance :
Must meet minimum requirements of the performance standards outlines in the criteria based evaluation and competency package.
