Charter Oak Health Care is seeking a dedicated and compassionate Community Health Worker to join our team. The ideal candidate will work closely with community members to improve health outcomes by providing education, support, and resources. This role involves outreach, advocacy, and collaboration with healthcare providers to address the social determinants of health.
Key Responsibilities :
- Conduct community outreach to identify individuals and families in need of health services.
- Provide health education and information to community members on various health topics.
- Assist clients in navigating the healthcare system, including scheduling appointments and accessing services.
- Advocate for the health needs of the community and connect clients with appropriate resources.
- Work with healthcare providers, social workers, and community organizations to ensure comprehensive care for clients.
- Maintain accurate records of client interactions and services provided.
