Physical Therapist Director

We are seeking a dedicated and compassionate Occupational Therapist to join our team. In this role, you will provide essential therapeutic services to patients across various settings, including home health, skilled nursing facilities, and acute care environments. Your expertise will help individuals regain independence and improve their quality of life through tailored interventions and support. The ideal candidate will have experience working with diverse populations, including pediatrics and individuals with disabilities or autism.

• Responsibilities
• Conduct comprehensive assessments of patients' functional abilities and needs.
• Develop individualized treatment plans based on patient evaluations and goals.
• Implement therapeutic interventions to enhance patients' physical, emotional, and cognitive skills.
• Provide education to patients and their families regarding treatment plans and home exercises.
• Collaborate with interdisciplinary teams to ensure coordinated care and effective discharge planning.
• Maintain accurate documentation of patient progress and treatment outcomes.
• Participate in case management activities to optimize patient care pathways.
• Stay updated on best practices in occupational health and therapy techniques.
• Requirements
• A valid Occupational Therapy license in the state of practice.
• Experience in patient service with a focus on home health or skilled nursing facilities is preferred.
• Knowledge of discharge planning processes and case management principles.
• Familiarity with working with individuals with disabilities, autism, or other special needs is a plus.
• Strong communication skills to effectively interact with patients, families, and healthcare professionals.
• Ability to adapt treatment approaches based on individual patient needs in acute care settings or outpatient environments.
• Commitment to ongoing professional development and adherence to ethical standards in therapy practice.

Pay : $60.00 - $65.00 per hour
