    Responsibilities :

  • Conduct patient assessments to determine physical condition, limitations, and personal goals.
  • Develop individualized treatment plans utilizing the latest research and techniques.
  • Implement therapeutic interventions effectively, tracking patient progress and adjusting treatments as necessary.
  • Provide patient education and support for ongoing health management.
  • Collaborate with a multidisciplinary team to ensure comprehensive care.
  • Maintain accurate documentation and patient records according to regulatory standards.

    • Requirements :

  • Bachelor s or Master s degree in Physical Therapy from an accredited program.
  • Current PT licensure in the state of Indiana or eligibility for licensure.
  • Minimum of 5 years clinical experience, with advanced skills in orthopedic, neurologic, or balance therapy highly preferred.
  • Demonstrated excellence in patient care and a strong commitment to clinical quality.
  • Excellent communication and interpersonal skills.
